ACH Automatic Transfer Authorization form: use when the transfer originates from a non-Heartland Bank account.
AFT Automatic Transfer Authorization form: use when the transfer originates from a Heartland Bank account.
Complete the form you need, print it, and send it one of two ways:
- Mail to: Heartland Bank, Attn: Loan Servicing Department, P.O. Box 67, Bloomington, IL 61704-0067
- Scan and e-mail by logging into Heartland Bank's Online Banking and using the "Secure E-mail" service.
